  • Charlottesville Healthcare Access provides health screening and healthcare navigation for the homeless and underserved in the Charlottesville and Albemarle County region of Central Virginia.

  • History:
  • Charlottesville Health Access (CHA) began in February 2007 when a private, non-profit community homeless shelter, PACEM (People and Congregations Engaged in Ministry) partnered with medical and nursing students and physicians from University of Virginia Schools of Medicine and Nursing (UVa. SOM and SON) to organize a health fair for shelter guests. Afterward, medical students began studying existing needs assessment data from the Thomas Jefferson Area Coalition for the Homeless and PACEM regarding the medical needs of the area’s homeless. These discussions lead to the formation of a coalition of service providers and recently homeless individuals that investigated ways of providing a more permanent solution to break down barriers to health care faced by the homeless in Charlottesville.
    From these discussions, Charlottesville Healthcare Access was formed with the mission of enabling homeless persons in the region to access the current healthcare system. First our homeless clients are triaged to determine their medical needs. Next, trained volunteers, termed navigators, meet with the clients and work with them to schedule appointments, understand health information, and complete financial screening forms. This system is in full operation at several rotating sites within the region. In addition to the navigator program, CHA created a prescription assistance program to help our clients obtain the medications that their physicians prescribe.
