  • Mission:
  • The mission of Chesapeake Health Investment Program (CHIP of Chesapeake and Portsmouth, or "CHIP") is to build a healthy community by building a healthy child. The agency strives to fulfill this mission by providing a continuum of support, education and referral services to young children and their families in Chesapeake and Portsmouth. Specific services include home visitation-based case management, children’s health assessments and health education, developmental screenings and referrals, one-on-one and group parent education, health insurance enrollment services, and transportation to medical and human services appointments. CHIP’s vision is that all children enter school healthy and ready to learn and succeed.

  • History:
  • CHIP is a 501(c)(3) non-profit integrated model program. The agency, part of the Comprehensive Health Investment Program of Virginia, focuses on improvement of children’s health through insuring that low-income children have medical homes and provision of resources and support services for their families. In addition, CHIP offers First Steps, a volunteer-based parent support, education and referral program, the “sleeptight” safe sleeping program, and a comprehensive Hispanic Outreach Program.

    Based on recommendation from CHIP of Virginia, CHIP of Chesapeake expanded its services to include Portsmouth in July of 2010. With the combination of the Chesapeake and Portsmouth programs, CHIP’s model of service delivery is implemented in order to increase the standard of service delivery and the number of children and families served in Portsmouth. The merger of these two programs leads to increased efficiencies in financial, human and facility resources – and more children will be served with quality home visitation and case management services.

    Incorporated in 1998 and with over 10 years of proven outcomes, CHIP has become the "go to" agency for comprehensive home visitation-based case management and nursing services for young children and their families. Its strong partnerships with Chesapeake General Hospital, Chesapeake Health Department, the Chesapeake Department of Social Services, United Way, Chesapeake Public Schools, the City of Chesapeake, the business community, the faith community, and the health provider community contribute significantly to the success of CHIP’s programs, and CHIP’s Board and staff are committed to developing these same strong organizational partnerships in Portsmouth. CHIP, however, is distinguished from other health and human services agencies in that its programs and services are strictly voluntary, it is based on prevention and early intervention, and it provides a continuum of services to children based on the strengths and needs of the family.
